Whether recovering from surgery or short-term illness, returning home after a knee or hip replacement, or living at home with chronic illness or a disability, our rehabilitation therapy services can help.
We work closely with the patient, their caregivers and their personal physician to develop a customized care plan that focuses on patient safety, regaining mobility and maintaining independence. In addition to the licensed therapist, your care team may include registered nurses, social workers and certified home health aides. Our program emphasizes continuity of care and provides a smooth transition from the hospital or rehabilitation facility to home. Each patient will receive a complete assessment to determine appropriate care and therapy needs.
After conducting a comprehensive evaluation of the functional level, the physical therapist will develop a care plan.The treatment will be carried out through a schedule of regular in-home visits.
A licensed occupational therapist, in conjunction other members of the home care team, will develop and implement an in-home treatment plan to retrain and rehabilitate essential life skills lost as a result of an illness or injury. This includes helping our patients achieve the highest level of independence in dressing, bathing, showering, feeding, and home/kitchen management/meal preparation. An Occupational Therapy evaluation looks at all aspects of the client: cognition, vision, sensation, strength, mobility as well as their roles in the home and their goals for returning to their previous functional level.
Licensed Speech-language pathologists work as part of the VNA of Somerset Hills patient care team to evaluate and diagnose speech, language, cognitive communication and swallowing disorders that are the result of illness or injury. Our therapist develops a plan of in-home care, including self-care and caregiver strategies, that will help improve these difficulties.
Rehabilitation therapy is an important and beneficial treatment for many acute and long-term conditions and can sometimes provide a positive alternative to surgery and/or pain medication. Some conditions that benefit include:
