Care Coordination: VNA case managers help coordinate a smooth transition home from the hospital or a rehabilitation facility. We ensure adequate communication between healthcare providers, hospital discharge planners, patients and caregivers.
Customized Care Plan: Working in conjunction with the patient and their physician, a registered nurse or therapist will create an individualized care plan of care and act as case manager for all home care services we provide.

Hospitalization is not a requirement for home health care. Many of our patients are referred by their personal physician.

Yes. Federal law gives all patients the right to choose their home health care provider.
Yes. Our agency is certified as a Medicare provider by the Centers for Medicare and Medicaid (CMS), accredited by Community Health Accreditation Program (CHAP), and licensed by the New Jersey Department of Health and Senior Services.
Our patients can be referred by their own physician, a hospital, rehabilitation facility, skilled nursing or assisted living facility, clergy, family, friends or self-referral. Before service can begin, the patient’s physician will write an order that prescribes home care. If you are referring yourself or a loved one, provide us with your physician’s name and phone number and our Referral Center staff will be happy to request the order. Once the VNA receives this order, we’ll contact the insurance company directly and determine if coverage is available and if authorization is required, notify the patient of any co-pays or deductibles, and schedule the first visit.
Home health care provides physician-prescribed skilled nursing and rehabilitation therapy services to individuals recovering from illness or surgery, living with chronic conditions and those who can avoid hospital admission and remain living safely at home with medical and personal care assistance. Personal care services provide for individuals who need assistance with Activities of Daily Living such as bathing, dressing, toileting, ambulation, or feeding meal preparation, grocery shopping, and money management.
To qualify for Medicare-covered services, a patient must have “homebound” status, which is defined as a general inability to leave home due to illness or injury. Leaving home requires taxing effort, and absences from home are infrequent, and for short duration. However, it does not mean that the person is confined to bed or cannot leave the home at all.
