At the VNA of Somerset Hills, we understand that home care and hospice patients and their families have many concerns that impact their emotional health and quality of life. Our compassionate staff of licensed medical social workers offers the support strategies necessary to help meet these needs. We provide important information and can recommend community resources to assist patients and family members with complex issues such as personal needs, long-term planning and financial concerns.
Our social workers communicate frequently with nurses, home health aides, therapists and the entire patient care team and are a resource for any social, emotional and environmental difficulties.
Family members, caregivers and friends of terminally ill patients often need assistance coping with their loss. Through our Hospice Care program, supportive counseling is available for 13 months following death, a timeframe helps deal with major holidays and anniversaries. We also offer a Bereavement Support Group to all members of the community who are grieving following the loss of a loved one.
