In Home Caregiver Education

Our In Home Caregiver Education is designed to provide education and support for unpaid caregivers. The program brings caregivers and professionals together in the home setting. One-on-one instruction is provided to enhance the caretaker’s ability and effectiveness and to maintain or restore safety as care giving changes over time.

What aspects of care giving does the program cover?

In a partnership with a Registered Nurse, caregivers will participate in an assessment of learning needs and development of a comprehensive teaching plan. The caregivers will then be able to identify and develop goals for learning those skills necessary to provide care including but not limited to:

  • Lifting
  • Personal Care
  • Nutrition
  • Transferring
  • Bathing
  • Disease Management Education
  • Accessing Community Resources
  • Long Term Planning
  • Evaluation for necessary medical equipment
  • Short term counseling

Who is eligible for the program?

In order to be eligible for the program, a caregiver must be:

  • An unpaid, primary care provider
  • A spouse, adult child, grandchild, other relative or friend
  • Living with the care recipient or making regular visits to assist with personal care activities

In order to be eligible for the program, a care recipient must be:

  • Over 18 and functionally impaired with a chronic disease or disability
  • In need of the assistance of a primary caregiver to adequately perform activities of daily living
  • A resident of Somerset County
  • Ineligible for reimbursement for this service from Medicare, Medicaid or private insurance

Services provided by this program may include:

  • 1 RN visit for assessments to develop a teaching plan
  • Up to 5 home visits for teaching, including staff from the following disciplines: RN, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work and Nutrition
