Social Worker – Home Health, Morris County – Per Diem

JOB SUMMARY: Provides social work service to home health and hospice patients and their families and consultation to staff members. Acts as a resource person in matters pertaining to complex personal, social, emotional and environmental difficulties that cause social and personal maladjustment or which predispose toward illness or interfere with obtaining maximum benefit from health care.

QUALIFICATIONS:

  • Completion of a master’s degree program in social work (MSW) at an accredited graduate school of social work
  • Current license to practice Social Work in the State of New Jersey (LSW or LCSW)
  • Current New Jersey Drivers License


EXPERIENCE:

  • A minimum of one year post master’s experience in a health care setting with a demonstrated comprehensive knowledge of social work skills

KNOWLEDGE & ABILITIES:

  • Ability to adapt skills to the home health & hospice setting
  • Knowledge of community resources
  • Effective communication skills both written and oral
  • Ability to establish and maintain constructive working relationships with staff and clients/families
  • Ability to maintain patient confidentiality

DUTIES & RESPONSIBILITIES:

  • Upon referral of a patient the social worker will assess, in a timely manner according to VNA protocol, patient and family functioning in relation to health care needs including financial, environmental social and emotional needs; and for Hospice and Palliative Care patients, assessment will also include spiritual needs and risk for complicated bereavement
  • Identify gaps in service delivery and assist in planning, developing and implementing programs to meet these needs within the agency and the community
  • Assist Hospice and Palliative Care team members to understand the significant social and emotional factors related to the care of the terminally ill patient and the family
  • Assist Home Care team members to understand the significant social and emotional factors related to the care of the chronically ill patient and the family
  • Keep abreast of trends and developments in the field of Social Work, Home Care, Hospice and Palliative Care by attending continuing education programs
  • Provide Geriatric Care Management services when requested
  • Participate on Quality Council activities and PAC when requested
