Community Health Nurse for VNA Community Health Services – Per Diem

JOB SUMMARY: Provides health maintenance and monitoring and health promotion programs at identified community locations. Programs include Board of Health home visits; township sponsored screening clinics, senior clinics, immunization clinics both for children and adults, van, Friendly Visitor and Med-alert assessments. Responsible for supervising certified home health aides. Consults with and refers families to appropriate personnel within the agency, school or other community services.

REPORTS TO: Clinical Manager, Community Health


  • Current license to practice as a Registered Nurse in the State of New Jersey or has a license pending as indicated by a permission-to-work letter from the New Jersey State Board of Nursing
  • A minimum of one year’s experience in nursing


  • Comprehensive knowledge and skill in current nursing practice
  • Understanding and knowledge of the nursing process
  • Ability to organize, manage and function in assigned areas
  • Ability to accept and utilize supervision
  • Ability to recognize needs and concern for people
  • Ability to establish and maintain constructive working relationships with individuals and families


  • Identify patients who have physical and emotional illnesses and/or disabilities through observation and analysis of records and help families secure appropriate medical, hospital and other needed care. Serve as case manager for clients assigned which includes development of care plans, coordination of needed services and observation and monitoring of the patient’s response to care and treatment.
  • Demonstrate, teach and supervise care to patients and families
  • Record observations, treatments given and other significant information
  • Maintain adequate records at all times
  • Consult about individual or family health problems with other professionals such as physicians, dieticians, social workers and therapists as needed
  • Promote continuity of patient care by planning and exchanging information with physicians and other health and social agencies as needed and other health and social service agencies as needed
  • Participate on agency committees as requested
  • Interpret the role of the nurse and they agency’s programs to the community at large
  • Carry out other duties as assigned
