RN – Referral Service Center Liaison- FT

Job Summary

We invite you to apply for employment at our highly regarded independent community healthcare agency. The VNA of Somerset Hills, NJ has a Referral Service Center Liaison RN opening with an attractive compensation and benefits package. We are seeking New Jersey Registered Nurses who have an interest in customer relations and coordination of services. This team member possesses strong critical thinking skills necessary for coordinating care from hospitals, sub-acute rehabilitation facilities and assisted living facilities to ensure a safe and timely transition to home. They are responsible for working with physician offices and discharge-planning teams to facilitate and expedite appropriate home care placements with the Visiting Nurse Association of Somerset Hills (VNASH) so patients receive home care services as expeditiously as possible. They are also responsible for receiving and reviewing telephonic and electronic referrals from other sources, including but not limited to: physicians, physician practices, insurance companies, patients/families, other agencies.

Reports to: Associate Director of Referral Services and Sales

Qualifications:

   Required:

  • Current license to practice as a Registered Nurse in the State of New Jersey
  • Excellence in critical thinking/communication/organization skills.
  • Computer proficiency in Outlook, Word and internet

    Preferred:      

  • Bachelor’s Degree preferred
  • Previous referral and/or intake experience
  • Health care marketing experience
  • Knowledge of Home Care Home Base

Duties and Responsibilities:

  • Interprets agency policy to patients and facility personnel.
  • Serves as a liaison between facilities and VNASH, including on-site facility visits
  • Answers calls to Referral Service Center, consults with physicians, nurses and other disciplines to obtain referral information and provides feedback to facility personnel.
  • Enters all preliminary patient referral information into computerized “point of care” system as needed. Inputs/processes referrals into the system in a timely and accurate manner
  • Communicates with internal departments and external customers regarding initial patient visits to ensure positive customer satisfaction.
  • Participates in quality improvement activities.
  • Builds professional relationships with patients, families, providers, and internal customers to ensure service excellence.
  • Demonstrates knowledge of computer systems, electronic health record, scheduling system and other technology utilized within the organization.
  • Assists in evaluation of patients referred by facility staff for home care to determine appropriateness for admission to VNASH programs. Works with discharge planning team at facilities to refer patients needing other sources of care or appropriate service.
  • Consults with physicians, nurses and other disciplines while establishing a coordinated home care plan prior to discharge.
  • Provides feedback to facility personnel, as needed and/or requested.
  • Evaluates hospice referrals for eligibility. Coordinates needed services for hospice admissions. Works with pharmacy to arrange medication delivery. Works with DME supplier to arrange equipment delivery.
  • Processes hospice referrals, which includes obtaining MD orders, referral entries, scheduling initial visits, and processing Hospice CTI and worksheets for MD completion
  • Collects and maintains statistical data of referred patients, as needed.
  • Collaborates with VNASH Referral Service Center regarding patient plan of care and needs.
  • Serves as a resource to facility staff on community resources and home care issues and best practices.
  • Carries out the mission, vision and pillars of the organization.
  • Other duties as assigned.

LPN -Referral Service Center Liaison – FT

Job Summary
We invite you to apply for employment at our highly regarded independent community healthcare agency. The VNA of Somerset Hills, NJ has a Referral Service Center Liaison LPN opening with an attractive compensation and benefits package. We are seeking New Jersey licensed LPN’s who have an interest in customer relations and coordination of services. This team member is responsible for accurately and expediently entering preliminary patient referral information into system and ensuring that Referral Services Center calls are answered in a timely manner. They demonstrate competence in multi-tasking and prioritizing work, which ensures that we exceed our internal and external customer expectations. The team member is also responsible for working with the discharge planning team at hospitals and health care facilities to facilitate and expedite appropriate home care placements with the Visiting Nurse Association of Somerset Hills (VNASH) so patients may be appropriately discharged from the facility and receive home care services as expeditiously as possible after discharge.

Required:

  • Current license to practice as a Licensed Practical Nurse in the State of New Jersey

Speech Language Pathologist (SLP) – Per Diem

An exciting opportunity exists for a licensed speech language pathologist (SLP) to join the Visiting Nurse Association of Somerset Hills, a highly regarded non-profit home care and hospice agency that has served Somerset and Morris Counties for 114 years. POSITION SUMMARY

The VNA of Somerset Hills is seeking a Per Diem SLP to serve patients in Somerset and Morris Counties, New Jersey.  Our SLPs provide speech therapy treatment services to the agency’s home health care, hospice or private care patients in their homes or in senior residential communities.

 

QUALIFICATIONS AND EXPERIENCE

  • Current license to practice speech therapy in the State of New Jersey
  • Current New Jersey Driver’s License
  • Minimum three years of satisfactory work experience in speech therapy
  • Prior home care experience preferred

JOB SUMMARY:
The Home Health Care RN position is the cornerstone of VNA of Somerset Hills’ clinical operations. RNs provide nursing care for those with illness and/or injury wherever the patient calls home. This care is achieved through care plan development and oversight including clinical nursing interventions, education, and supervision of care plan implementation for patients and caregivers. Our RNs also provide consultation and referral to appropriate personnel and services within the agency and the community.


PRIMARY QUALIFICATIONS:

  • Comprehensive knowledge and skill in current nursing practice and an understanding and knowledge of the nursing process
  • Knowledge or interest in home care philosophy and practice
  • Completion of a baccalaureate degree in nursing from a college or university accredited by the National League for Nursing is preferred OR graduation from an accredited school of nursing
  • Current license to practice as a Registered Nurse in the State of New Jersey or has license pending as indicated by a permission-to-work letter from the New Jersey State Board of Nursing
  • Current New Jersey Drivers’ License
  • I.V. certification is preferred
  • Basic computer skills required

EXPERIENCE:

  • A minimum of one year’s experience in nursing
  • Home care experience preferred

On Call Nurse – Per Diem

JOB SUMMARY: As an On Call Nurse, you will be responsible for responding to patient/family phone calls and making visits as needed during off hours of the weekday and/or weekend. You will respond to the needs of hospice patients as well as homecare patients requiring 24 hour availability. The On Call Nurse may also need to establish patients on off hour admissions.

QUALIFICATIONS:

  • Graduation from an accredited school of nursing or college, bachelor’s degree preferred
  • Current NJ State nursing license or license pending as indicated by a permission to work letter from the NJ State Board of Nursing
  • Current New Jersey Drivers’ License
  • Minimum 2 years’ experience in Home Care or Hospice Nursing preferred
  • Comprehensive knowledge and skill in current nursing practice
  • Understanding of hospice philosophy and practice
  • Excellent communication and judgment skills to enhance telephone triage
  • V. certification preferred
  • Prior experience utilizing EHR, HomecareHomebase a plus

DUTIES & RESPONSIBILITIES:

  • Demonstrate, teach and supervise care to patients and families
  • Respond to all calls within a maximum timeframe of 15 minutes
  • Consult about individual or family health problems with the Administrative Call Nurse or physician
  • Promote continuity of patient care by planning and exchanging information with physicians and other appropriate team members
  • Participate on Hospice Team and in other team meetings, including presentation of cases, as needed
  • Rotate weekend visit responsibility: one weekend in every quarter year
  • Rotate one major and one minor holiday visit responsibility per year

Sales Account Manager – FT

JOB SUMMARY:

Are you a motivated and experienced field sales professional looking to become part of the growing home health industry? Join the Sales & Marketing team at the VNA of Somerset Hills, a nonprofit healthcare provider servicing Morris and Somerset counties. We deliver quality home health and hospice care to our patients, wherever they call home. Our comprehensive team of skilled health professionals includes registered nurses, physical, occupational and speech therapists, nutritionists, social workers and home health aides. Our focus is providing the care and support necessary to help individuals and their caregivers maintain their health, quality of life and independent living.

As a member of our team, you will be responsible for growing home health and hospice referrals by developing and maintaining relationships with referral partners, including physicians, skilled-nursing facilities and senior residences.

QUALIFICATIONS:

■ Field sales experience
■ Bachelor’s degree in business, marketing or health sciences related field, preferred
■ Minimum of three (3) years sales experience in a service industry, preferred
■ Home health and/or hospice sales experience is a plus
■ Microsoft Office required; Playmaker or CRM software experience is a plus

DUTIES & RESPONSIBILITIES:

■ Develop relationships with existing and prospective referral partners within assigned territory
■ Design and execute a comprehensive sales plan that aligns with agency strategic and KPI goals
■ Contribute to regular meetings with the VNASH marketing department, skilled clinical staff and other sales account managers to report on and develop tactical plans that target physicians’ practices, assisted living facilities, skilled nursing facilities, rehab facilities and hospitals
■ Ensure compliance with all privacy and healthcare laws and regulations
■ Other duties as assigned
